We are the Multidisciplinary Team (MDT), made up of professionals who will be providing treatment, advice and support for you and your family.
We aim to provide the highest possible standard of care and support for you.
As a team we aim to meet weekly to discuss each individual case, specifically looking at investigations, reports, diagnosis, treatment options and support plans.
Investigations and treatment may take place at hospitals across Lincolnshire and Nottinghamshire. Key members from each of these centres are represented at our weekly meeting and involved in care and treatment decisions.
Each patient is individual, as such they have different needs, requiring specialised care and support. This will form part of your Holistic Needs Assessment (HNA).
