What can we offer?

We can visit you at home or elsewhere if you prefer, to identify your needs and help ensure the right services are in place.

We also offer appointments by telephone or video.

If you are in hospital, we can offer an in-reach service to help support you whilst on the ward and to support discharge planning.

If you are admitted to hospital, please ask a relative or a nurse on the ward to inform the MND Coordinator on 01522 573995.

Sometimes you may be referred to us whilst you are still having investigations, where a diagnosis of MND is being considered, but not confirmed. In this case, we can help support you through this process and ensure that you have access to the services you need.

We have a dedicated email and phone number for you to contact the MND Coordinators directly in case of the need for support and advice.

We will work closely with anyone who might be involved in your care, this might include:

Areas Covered

We can support anyone with a Lincolnshire GP.
