Specialist Family Practitioner Team, Pre and Post Bereavement

Who are we

The Specialist Family Practitioner Team, Pre and Post Bereavement, cover Lincolnshire. We are a qualified team, adhering to professional standards and codes of ethics, which includes child and family bereavement nurse specialists with a play specialist and youth worker.

What we do

We help children, young people under the age of 19 years old and their families in understanding their thoughts and feelings as they navigate the impact of illness and bereavement, whilst offering a confidential safe space.

How to make a referral

We accept referrals from any professional who knows of families who meet our service criteria. Families who meet our criteria can also refer themselves via telephone.

A referral form must be completed. Families must have given consent before referrals are made.

Criteria

  • Child with a palliative or end-of-life diagnosis.
  • Children and young people who have a parent, guardian or sibling with a terminal illness and prognosis of less than one year.
  • Bereaved children and young people following the the death of a parent or guardian or sibling.

Please note the service is unable to offer

The service is unable to provide individual support for:

  • Families experiencing grief due to stillbirth or miscarriage
  • Families grieving the death of an extended family member (grandparent, aunt or uncle)
  • Adults with non-palliative oncology diagnosis or long term conditions
  • Families affected by separation or divorce

How we work

We are an integral part of the Community Children’s Nursing Services, working closely with other professionals involved with the child, including children’s agencies and organisations.

Visits are arranged in the family home, schools or other locations in the community. The service is available Monday to Friday, 9am to 5pm, excluding bank holidays.

Other services

Other services are available to ULHT Children’s Community staff, including; supervision, training and debriefs.

The team also offer telephone support, information and signposting for other professionals needing advice on child bereavement, plus teaching and education around this subject.
