Following wide consultation with adults who use our services, families, carers, and professionals, we are pleased to announce the relaunch of the All About Me booklet – refreshed, more person-centred, and designed with your input at its heart.

We asked lots of people what they thought including adults who use services, families and carers and staff and professionals

We listened carefully to feedback from those who use this tool and those who support them.

What is the booklet for?

The booklet helps you tell others:

  • What is important to you
  • How you like to be supported
  • What makes you happy
  • What you want in your life

What has changed?

We have made the booklet:

  • Easier to understand
  • More flexible – you can choose what to fill in
  • Better for different people and situations
  • More inclusive – everyone should feel it is for them
  • Additional internal pocket to insert important documents

Who can use it?

  • Adults who get care or support
  • People living in supported housing or residential homes
  • People using health and social care services

How can you use it?

You can use the booklet:

What is All About Me?

All About Me is a Lincolnshire Community and Hospitals NHS Group communication tool designed to enable patients who have difficulty communicating their needs, to ensure such needs are known to health and social care staff. All About Me is patient held, completed by or on behalf of the patient and is there to inform care and care planning not to replace organisational documentation.

Where can I get an All About Me booklet?

There is a single point of contact for Lincolnshire for the All About Me via [email protected]

Do I need an All About Me booklet?

This is entirely your choice. The aim of the All About Me is to provide a voice and a means of communication for individuals who may have difficulty expressing the care needs in a health and social care setting. There is no obligation on anyone to use All About Me.

When do I complete my All About Me booklet?

Ideally your All About Me details should be completed at home.

How do I make staff aware I have an All About Me booklet?

When you arrive, please show your All About Me document to the staff and keep it handy and visible.

Do I have to fill in all parts of All About Me?

You may choose to complete just parts of All About Me document rather than every section. Whatever is right for you and for whatever you feel health and care staff need to know.

Do I give my All About Me to staff?

You can give them your All About Me booklet to begin with to help them understand your needs and to transcribe some of this information onto care plans. However, please make sure it goes home with you.

Will I get my All About Me back?

All About Me is YOURS – and it should stay with you wherever you are receiving care at all times – do not give to staff to keep.

If you are in hospital, please leave it somewhere visible.

Remember to pack it when you are ready for home.

My partner / relative can’t fill in their All About Me booklet – is it ok for me to do it?

Yes of course, you are an important part of their life and as such are supporting them to ensure they receive the right care and care the way they need it. Completing All About Me on their behalf is entirely acceptable.

I am using another type of hospital passport, can I use it?

There are other hospital passports that people use. It doesn’t matter which type of hospital passport you bring with you. The important thing is that you share useful information with staff.

My family want to fill in my All About Me for me and update it but they live a long way from me. Can it be done online?

Yes, we have an editable PDF version that you can be downloaded and completed by your family.  The document can be updated at anytime and resaved.  Family will then be able to share the completed All About Me electronically.

Who is coordinating and issuing All About Me?

The All About Me is centrally coordinated by United Lincolnshire Teaching Hospitals NHS Trust for the Lincolnshire Community and Hospitals NHS Group.

How will staff evaluate if individuals are benefitting from their All About Me?
