The Acute Liaison Nurse Team supports all people with a diagnosis of a Learning Disability or Autism that need to come to United Lincolnshire Teaching Hospitals NHS Trust for healthcare.
All referrals for patients with learning disabilities accessing ULTH Hospitals (Lincoln County Hospital, Grantham and District Hospital and Pilgrim Hospital, Boston), should be emailed to [email protected] or called through to 01522 307129. If you are calling outside of working hours (Monday to Friday 9am-5pm) please leave a message on the answer machine, please include the following details:
- Patients full name
- Date of birth
- NHS number (if possible)
- Reason for requesting ALN support
Details of the referral will then be triaged by the ULTH Safeguarding Administrative Team. All triaged referrals will be transferred to the most appropriate Acute Liaison Nurse. Please note the team have 48 hours (Mon-Fri) to respond, however we aim to respond as soon as possible.
