Criteria for referral to the service
We provide care for the child or young person in their home whilst their parent/ carer have a short break from caring. All new referrals will receive an initial assessment visit from the team leader within two weeks from receipt of the completed referral form. All new referrals are reviewed at the monthly team meeting to allocate a children’s palliative care support worker and hours to support the family.
Please note: this is a limited resource service, therefore takes into consideration existing support already provided to the family. Continuing care support is available to support those children/young people with highly complex healthcare needs.
Referrals will be accepted from the child or young person’s children’s community nurse.
Please refer the following children and young people:-
- A child or young person with a progressive life-limiting condition that is not expected to live into adulthood.
- A child or young person who is currently palliative and/ or are likely to die within 1 year.
Children/ young people who are not expected to meet the criteria for this service
- Children living with significant health needs and/or disability, but whose risk of dying is comparable to that of the general population.
- Children who recover from an episode of critical illness and whose risk of dying is comparable to that of the general population.

The referral form must be completed by the referring professional with the child/ young person’s and parent/ carer. The adult with parental responsibility must sign the referral form to consent to the referral being made.
