The New Springwells Practice is a caring Practice providing quality health care in a rural setting. The Practice patient population is 5830 patients and employs 23 staff. The Surgery at Billingborough was purpose built in 1984 and recently extended in 2009.

The Billingborough Surgery has 6 well-equipped consulting rooms, two nurse treatment rooms, and large dispensary with reception area. A small branch surgery at Rippingale consists of a consulting room, a nurse treatment room, small dispensary and reception area. The Rippingale Surgery is open on a part-time basis. This practice has been training for more than 30 years and has always been popular with GP specialty registrars. The rota is designed to give GP registrars plenty of experience in a protected atmosphere. After a few days' introduction to the practice, most GP specialty registrars wish to hold their own light surgeries and, before long, they would expect to see about 70 patients per week. After an early assessment of the GP registrar's educational needs, a training curriculum is developed. There is formal teaching weekly and informal contact and teaching daily. Video recording of consultations and sitting-in occur regularly. There is a nominated GP registrar room. There is adequate time for study, and GP registrars have access to a wide range of books and journals. Registrars also have access to the internet via their consulting room PC. This is a purpose-built new surgery in the centre of Boston. The practice is fully computerised, and statistics for all targets and clinics are generated from the computer. It runs its own anticoagulant and phlebotomy clinics, sigmoidoscopy sessions, audiometry and tympanometry sessions, minor surgery sessions and has its own slit lamp. The trainer provides one tutorial per week, combined surgeries, debriefing sessions and analysis of video-recorded consultations. The practice aims to provide high-quality training combined with plenty of experience and maximum enjoyment.

These practice premises are second to none. The building is spacious and well equipped. The practice has been training for 12 years. All partners and attached professionals will be involved in the teaching programme. Any new GP specialty registrar will be assessed early on in the attachment to establish their educational needs, and a training curriculum will be developed. There will be regular weekly formal teaching sessions, along with more informal daily contact. The GP specialty registrar will have his/her own consulting room. The practice premises are modern and purpose-built with room for expansion and increasing services in both traditional and alternative therapies. The practice offers a full range of clinics and is in receipt of full higher-level immunisation and cytology targets. It provides minor surgery, as well as offering nurse-led chronic disease management clinics, supplemented with various GP input. The practice is committed to strong clinical governance ethic and closes every Thursday afternoon for in-house training, including clinical, organisational and significant event reviews.

The current practice premises were built in 1997 and have since been extended in order to provide more space and services. It has a constructive "can do" culture, which is supportive and values getting the job done. There are regular clinical and organisational meetings. The form of training in the practice is to assess a registrar's learning needs and then to agree a training plan. This will usually involve twice-weekly tutorials and informal case discussion following clinics. Registrars are encouraged to develop at their own pace towards an objective of being a competent, confident, independent practitioner.

This practice has just moved into huge brand new premises with 30 fully-equipped consulting rooms, two group training rooms, a library, minor ops suite, a patient information centre and individual rooms for two GP registrars with videotape equipment installed, as well as a fitness suite and physio gymnasium. Also on site are housed: district nurses, physiotherapist, midwife, pharmacy, ambulance station, dentist, the PCT and consulting suites for visiting consultants in ENT, rheumatology, paediatrics, gynaecology, urology, general surgery, orthopaedics and ultrasound and are looking to provide mobile X-ray next year. The practice has upgraded from EMIS to using "System One" computer system, which allows integration in communications with hospital and attached staff such as district nurses etc. Dr Cass is also a Boston GP Specialty Training Programme Director.
