Transitional Homecare Team
What is the homecare team?
We are a team of specialised neonatal nurses based at Lincoln County Hospital and Pilgrim Hospital, Boston providing a new service for parents taking their baby home from our Special Care Baby Units (SCBU). Our aim is to provide parents with support after their baby is discharged.
We are available to:
- Visit families in their home
- Perform any necessary tests or observations in the home
- Liaise with GPs and health visitors
- Empower families by teaching them the skills they may need before discharge, including basic resuscitation, benefits of breastfeeding and immunisation and health promotion
Will the team be available to all babies leaving the SCBU?
The team is available to families identified as needing extra support after discharge from SCBU. This may include babies:
- Who have been born premature
- Who need help to establish feeding
- Who require oxygen therapy
- With complex problems or special needs
Why is this service required?
Although many parents are overjoyed at the prospect of taking their baby home from SCBU, they often feel stressed and anxious about looking after their baby without backup from the hospital staff.
The team will support and educate parents on the care of these babies after discharge. The input provided by the team will enable earlier discharge of premature babies and provide a seamless pathway from SCBU to home.
