What is Cardiac Rehabilitation?
Phase 4 - Long Term Maintenance
This is the long term maintenance phase, therefore it needs to be the element of your recovery that you feel you can maintain for the rest of your life. We hope that you have been provided with the necessary confidence and motivation to maintain your healthier lifestyle and are obviously keen to support you in achieving this! We are fortunate in Lincolnshire to be able to offer you referral to several Phase 4 exercise schemes which are delivered in various settings within the community. Phase 4 Cardiac Rehabilitation Programmes are delivered by exercise professionals who have completed a specialist training course in delivering cardiac rehabilitation. (British Association of Cardiac Rehabilitation Phase 4) These programmes enable you to receive specialist advice on increasing your exercise tolerance and also ensure that this is done in a safe environment and at an appropriate pace.

Should you however on completion of Phase 3 fancy a change from the gym environment, we hope to be able to signpost you towards the many alternative options available to you!
Although for most this is the end of their journey through the cardiac rehabilitation process, we aim to maintain an open door policy ensuring that ongoing advice and support is available at the individuals request. All patients are encouraged to contact their cardiac rehabilitation nurse specialist should they require any further advice or support about problems or concerns in the future.
