What is Cardiac Rehabilitation?
Phase 2 - Immediate post discharge period
Phase 2 takes place during the immediate post discharge period, this is a time when patients often feel quite isolated and vulnerable. Patients will receive an initial telephone call to review their progress since discharge home. For those patients living in Lincolnshire they are also offered the option of a home visit from the community cardiac rehabilitation nurse specialist.
During the home visit your progress will be discussed and your nurse will ensure that an individualised care package is in place prior to their departure.
Topics covered will usually include:
- Further advice on risk factor modification
- Additional information and suggestions on maintaining a healthy lifestyle
- Further discussion about your diagnosis and treatment plan
- An opportunity to clarify any concerns you may have regarding your medication
- A review of current activity levels and suggestions on how you might safely increase these further over the next few weeks
- Further information regarding vocational and social issues and where necessary referral to specialist services
- An opportunity for the nurse to check your Blood Pressure and Heart rate
- Discussion regarding your ongoing cardiac rehabilitation requirements and provision of information about your local options for Phase 3 and Phase 4
- The nurse will then feedback to your GP to update them on your progress since discharge home and your ongoing cardiac rehabilitation plans.
- They will also ensure that your phase 3 provider is aware of your ongoing cardiac rehabilitation needs therefore enabling swift transition into the next phase of your recovery.
